Welcome to Advanced Physical Medicine & Rehabilitation! We’re delighted that you’ve chosen our practice for your natural healthcare needs. If you’ve been to other doctors’ offices, you may have felt they were cold or clinical. Our practice is warm and laidback, not stuffy, yet always professional.
Before You Arrive
You can save time by completing your new patient paperwork before you arrive. You can download it here or we’re happy to email it to you. You also can fill it out in the office. We just ask that you arrive 10-15 minutes early. Please bring your insurance card. You don’t have to wear anything in particular.

One of our team members will greet you. Then you’ll be escorted to our exam room. You’ll meet with Dr. Clark. He will review your health history. Next, he will conduct a comprehensive orthopedic exam and provide you with some form of therapy. You may receive electro-stim therapy or laser therapy.
If Dr. Clark determines that everything is clear from your exam, he will give you your first chiropractic adjustment. In some cases, Dr. Clark needs to get imaging. If he determines that you need X-rays or MRIs, he will send you to get those done. Then, when the images are ready, he’ll ask that you come back for a second visit to go over them to better establish a treatment plan.
When you go to check out, your care plan will be at the front desk, so a team member can get you scheduled for future appointments.
Please allow about an hour for this initial appointment.
As our plans are customized for each patient, the number of visits will vary from person to person. Rest assured that we will never try to push any lifetime care plan on you. Our goal is to get you better. Then, if you decide to come in for wellness or maintenance are we can discuss that once you’re better.
Financials
If we can get your insurance verified at your first appointment, we will provide details about your care costs. Otherwise, we will give you that information at your next visit. We offer CareCredit and convenient payment plans.
